This form is required to give another adult permission to bring and be responsible for your children at KACH activities in your stead. You do not need to fill this form out to bring your own children to KACH events.
If there are special circumstances where multiple events or permission needs to be granted for the entire program year, make sure to reach out to board@kachconnect.com first.

In case of emergency, I hereby authorize this person to immediately seek medical treatment as they deem necessary. I understand that I am responsible for all medical costs incurred.
